    Promoting adherence to antiretroviral therapy: the experience from a primary care setting in Khayelitsha, South Africa.

    OBJECTIVE: To describe the approach used to promote adherence to antiretroviral therapy (ART) and to present the outcomes in the first primary care public sector ART project in South Africa. DESIGN: The study is a prospective open cohort, including all adult patients naive to previous ART who received antiretroviral treatment in Khayelitsha, from May 2001 to the end of 2002. Patients were followed until their most recent visit before 31 July 2003. METHODS: Plasma viral load was determined at 3, 6, 12, 18 and 24 months after ART was initiated, and CD4 cell counts 6-monthly. Kaplan-Meier estimates were determined for the cumulative proportions of patients surviving, and patients with viral load suppression and viral rebound. RESULTS: A total of 287 patients were initiated on triple therapy. The probability of survival was 86.3% at 24 months. The median CD4 cell count gain was 288 cells/microliters at 24 months. Viral load was less than 400 copies/ml in 89.2, 84.2 and 69.7% of patients at 6, 12 and 24 months, respectively. The cumulative probability of viral rebound (two consecutive HIV-RNA measurements above 400 copies/ml) after achieving an HIV-RNA measurement below 400 copies/ml was 13.2% at 18 months. CONCLUSION: The study shows that, with a standard approach to patient preparation and strategies to enhance adherence, a cohort of patients on ART can be retained in a resource-limited setting in a developing country. A high proportion of patients achieved suppression of viral replication. The subsequent probability of viral rebound was low

    The effect of estrogen and tamoxifen on hepatocyte proliferation in Vivo and in Vitro

    We have previously shown that changes in estrogen‐hepatocyte interaction occur during liver regeneration. Following 70% hepatectomy, estrogen levels in the blood were elevated, the number of estrogen receptors in the liver was increased and there was an active translocation of estrogen receptors from the cytosol to the nucleus. The injection of tamoxifen, an estrogen antagonist, inhibits hepatocyte proliferation following partial hepatectomy. The administration of 1 μg tamoxifen per gm body weight at zero time or 6 hr after the operation resulted in a significant inhibition both of DNA synthesis and of the number of cells in mitosis. Injections of tamoxifen 12 hr or later after the operation had no effect. Concomitant injections of equimolar amounts of estrogen abolished the inhibition by tamoxifen. The effects of estrogen and tamoxifen were also tested on hepatocytes in primary culture. Estrogens in the presence of 5% normal rat serum stimulated hepatocyte DNA synthesis as determined by [3H]thymidine incorporation and the labeling index, whereas epidermal growth factor‐induced DNA synthesis in the absence of normal rat serum was strongly inhibited. Tamoxifen, in contrast, inhibited DNA synthesis of hepatocytes in the presence of 5% normal rat serum and reversed the stimulatory effect of estrogen in the same system. Attempts to elucidate the mechanism of tamoxifen inhibition in vitro indicated that one effect of tamoxifen is to prevent the amiloride‐sensitive Na+ influx necessary to initiate hepatocyte proliferation.     Towards a Robuster Interpretive Parsing

    The input data to grammar learning algorithms often consist of overt forms that do not contain full structural descriptions. This lack of information may contribute to the failure of learning. Past work on Optimality Theory introduced Robust Interpretive Parsing (RIP) as a partial solution to this problem. We generalize RIP and suggest replacing the winner candidate with a weighted mean violation of the potential winner candidates. A Boltzmann distribution is introduced on the winner set, and the distribution’s parameter TT is gradually decreased. Finally, we show that GRIP, the Generalized Robust Interpretive Parsing Algorithm significantly improves the learning success rate in a model with standard constraints for metrical stress assignment

    ELECTRICITY INFRASTRUCTURE PLANNING: EVALUATING SOLAR POTENTIAL ASSESSMENTS FOR INFORMAL SETTLEMENTS USING GRASS AND FREELY AVAILABLE DATA

    Informal settlements, also known as slums or shanty towns, are characterised by rapid and unstructured expansion, poorly constructed buildings, and in some cases, they are on disputed land. Such settlements often lack basic services, such as electricity. As a result, informal settlement dwellers turn to hazardous alternative sources of energy, such as illegal electricity connections and paraffin. Solar power is a clean and safe alternative. However, informal settlements are often located on undesirable land on the urban fringe where the topography may hinder the use of solar energy. The high density of dwellings could also be a hindrance. Therefore, the solar potential needs to be assessed before any implementations are planned. Solar potential assessment functionality is generally available in geographic information system (GIS) products. The nature, cost and accessibility of datasets required for the assessment vary significantly. In this paper, we evaluate the results of solar potential assessments using GRASS (Geographic Resources Analysis Support System) for a number of different datasets. The assessments were done for two informal settlements in the City of Tshwane (South Africa): Alaska, which is nestled on a hill; and Phomolong, a densely populated settlement with a rather flat topography. The results show that solar potential assessments with open source GIS software and freely available data are feasible. This eliminates the need for lengthy and bureaucratic procurement processes and reduces the financial costs of assessing solar potential for informal settlements

    LANDFORM PERCEPTION ACCURACY IN SHADED RELIEF MAPS: A REPLICATION STUDY CONFIRMS THAT NNW LIGHTING IS BETTER THAN NW AGAINST THE RELIEF INVERSION EFFECT

    Relief inversion effect is a perceptual phenomenon that leads to an inverted perception of convex and concave shapes. This perceptual inversion occurs in scenes where the shading/shadows act as the main depth cue. In visuospatial displays, such as shaded relief maps, the positioning of the shadows in the northern slopes, thus when light source placed broadly in south, mislead the cognitive system based on the ‘light from above prior’ assumption (Mamassian and Goutcher 2001). Thus, assuming the light must come from above, our mind creates an illusion, and we perceive the landforms incorrectly. To judge the 3D spatial relationships in terrain representations correctly, the relief inversion effect must be avoided. Cartographic convention against this effect is to place the light source at northwest (NW), whereas a recent study demonstrated that north-north-west (NNW), or even north yields more precise results (Biland and Çöltekin, 2016). Since this finding goes against decades of convention, to establish its validity further, we attempted replicating the results with a different sample in South Africa. In this paper, we present our findings, which broadly confirm that the NNW (or also N) is better than NW against the relief inversion effect

    Indoor collections of the Anopheles funestus group (Diptera: Culicidae) in sprayed houses in northern KwaZulu-Natal, South Africa

    BACKGROUND: Insecticide resistance in malaria vector mosquitoes presents a serious problem for those involved in control of this disease. South Africa experienced a severe malaria epidemic during 1999/2000 due to pyrethroid resistance in the major vector Anopheles funestus. Subsequent monitoring and surveillance of mosquito populations were conducted as part of the malaria vector control programme. METHODS: A sample of 269 Anopheles funestus s.l. was collected in Mamfene, northern KwaZulu-Natal, using exit window traps in pyrethroid sprayed houses between May and June 2005. Mosquitoes were identified to species level, assayed for insecticide susceptibility, analysed for Plasmodium falciparum infectivity and blood meal source. RESULTS: Of the 220 mosquitoes identified using the rDNA PCR method, two (0.9%) were An. funestus s.s. and 218 (99.1%) Anopheles parensis. Standard WHO insecticide susceptibility tests were performed on F1 progeny from wild caught An. parensis females and a significant survival 24 h post exposure was detected in 40% of families exposed to 0.05% deltamethrin. Biochemical analysis of F1 An. parensis showed no elevation in levels/activity of the detoxifying enzyme systems when compared with an insecticide susceptible An. funestus laboratory strain. Among the 149 female An. parensis tested for P. falciparum circumsporozoite infections, 13.4% were positive. All ELISA positive specimens (n = 20) were re-examined for P. falciparum infections using a PCR assay and none were found to be positive. Direct ELISA analysis of 169 blood meal positive specimens showed > 75% of blood meals were taken from animals. All blood fed, false positive mosquito samples for the detection of sporozoites of P. falciparum were zoophilic. CONCLUSION: The combination of pyrethroid resistance and P. falciparum false-positivity in An. parensis poses a problem for vector control. If accurate species identification had not been carried out, scarce resources would have been wasted in the unnecessary changing of control strategies to combat a non-vector species
